The allegations read: The client asked me to sign her name, which i did, to an annuity contract so that she could lock in a date for dollar cost averaging. Nothing was to happen until i returned from vacation but the forms were inadvertently processed (this can be confirmed by the annuity company). She wrote a letter of complaint requesting that the old positions be reestablished which was done at no loss.
