The allegations read: Allegations against co-defendant equitable life are that equitable life arbitrarily terminated monthly benefit payments under a disability income policy purchased by
[customer] even though she allegedly remains disabled. Damages sought are the monthly payment of $5,000.00 per month from march 1998 plus interest. Although i am named as co-defendant, there are no allegations against me. I am an agent of equitable life.
